Takeaway

  • Men with osteoporosis had a higher risk of all-cause mortality and a higher incidence of and mortality from cardiovascular disease (CVD) and respiratory diseases, including chronic obstructive pulmonary disease (COPD).

  • Osteoporosis had significant associations with CVD incidence, respiratory disease incidence and mortality, COPD incidence, and breast cancer mortality in women

Why this matters

  • Findings suggest that screening for associated cardiovascular and respiratory diseases should be incorporated in the management of osteoporosis, particularly in men.

Study design

  • An analysis of 305,072 participants (age, 37-73 years; women, 53%), identified from the UK Biobank (2007-2010).

  • The association of osteoporosis with the incidence of and mortality from CVD, respiratory disease, cancer, and all-cause mortality was evaluated.

  • Funding: None disclosed.

Key results

  • After adjustment for potential confounders, men with osteoporosis had a higher risk of (adjusted HR [aHR]; 95% CI):

    • all-cause mortality (1.71; 1.38 to 2.11; P < 0.0001);

    • CVD mortality (1.68; 1.19 to 2.37; P = 0.003);

    • respiratory mortality (2.35; 1.70 to 3.24; P < 0.0001);

    • respiratory incidence (1.26; 1.06 to 1.50; P = 0.009);

    • COPD mortality (3.64; 2.24 to 5.91; P < 0.0001); and

    • COPD incidence (1.82; 1.38 to 2.40; P < 0.0001).

  • Women with osteoporosis had a higher risk of (aHR; 95% CI):

    • CVD incidence (1.24; 1.07 to 1.44; P = 0.004);

    • respiratory mortality (1.31; 1.00 to 1.72; P = 0.047);

    • respiratory incidence (1.23; 1.13 to 1.33; P < 0.0001);

    • COPD incidence (1.29; 1.10 to 1.52; P = 0.002); and

    • breast cancer mortality (1.60; 1.14 to 2.26; P = 0.007).

Limitations

  • Risk of healthy volunteer selection bias.
