Clinical Trials Comparing Earlier Versus Delayed Kidney Replacement Therapy Initiation in Acute Kidney Injury
Earlier observational studies and small randomized controlled trials reported a better survival of patients who commenced KRT earlier.[22–26] Nevertheless, these studies had important methodological limitations. Four recently completed randomized controlled trials compared the effect of KRT initiation strategies on mortality in critically ill patients with severe AKI. These RCTs have provided us with new perspectives on the optimal timing of initiation for acute KRT significantly improving the current level of evidence and our understanding on this subject (Table 1).
