A 35-year-old man with a history of hypertension treated with a beta-blocker presents to his primary care provider for a routine physical examination. He has no complaints, and physical examination is unremarkable except for some variability in the intensity of S1 and evidence of cannon A waves in the neck. An ECG is obtained.
