ECG Challenge: An Unexpectedly Rapid Heart Rate in the Emergency Department

A 56-year-old woman presents to the emergency department (ED) after falling from her bicycle. She has severe pain in her left wrist, and an x-ray demonstrates a fracture. While waiting for a cast, she is noted to have episodes of a rapid heart rate, and a 12-lead ECG is obtained (Figure 1).
