Topical NSAIDs

NSAIDs are also available as topical formulations, most commonly diclofenac. They have little systemic absorption, along the order of less than 1.5% peak concentration compared with a 50-mg diclofenac tablet.[33] Hence, they are safe to use from a cumulative toxicity perspective. Isolated cases of interstitial nephritis have been reported, which are plausible given that they may be idiosyncratic, dose independent reactions.[34] The question of efficacy of topical NSAID preparations is separate.[35] It may or may not be from the NSAID itself, and there may be a contribution from the non-NSAID counterirritant ingredients.
