The diagnosis of SIJ pain is a clinical and radiographic challenge. Currently, fluoroscopically guided intra-articular injection is the benchmark for diagnosing this condition. Patients' history, the presence of positive provocative tests, and possible imaging findings may implicate the SIJ as the source of pain, and these patients should be considered for an SIJ injection, which may be diagnostic and therapeutic. If the injection leads to at least 75% pain relief, the SIJ is the likely pain generator.
