Case Challenge: Is This Just Another Pigmented Lesion?

Clinical Presentation

The patient is a 22-year-old man with dysplastic nevus syndrome. He has had multiple nevi excised over the past 2 years because of irregular appearance. Today he presents with an irregularly pigmented growth with a pedunculated type stalk, located just below the right areola (Figure 1).

To the patient, this growth seems different from the other "moles" that have been removed in the past. He can feel a slight lump under this growth. The rest of his history and physical examination are unremarkable.
