When is thrombolytic therapy indicated in the treatment of acute ischemic stroke (AIS)?

Despite the increased risk of hemorrhage in patients with a massive stroke, fibrinolysis remains indicated whenever other exclusion criteria are absent. The potential benefit is tremendous in this population of patients, who almost always will have a dismal outcome if therapy is withheld. Inclusion and exclusion criteria must be reviewed before administration of a thrombolytic agent. Be aware of SAH that presents early without CT findings.
