While antiretroviral therapy (ART) is recommended for all patients with HIV infection, the following conditions increase the urgency to initiate therapy: [2]

  • Pregnancy
  • AIDS-defining conditions
  • Acute opportunistic infections
  • Lower CD4 counts (eg, < 200 cells/µL)
  • Rapidly declining CD4 counts (eg, >100 cells/µL decrease per year)
  • Higher viral loads (eg, >100,000 copies/mL)
  • HIV-associated nephropathy (HIVAN)
  • Acute/early HIV infection
  • HIV/hepatitis B virus coinfection
  • HIV/hepatitis C virus coinfection
