Answer

For STEMI with onset of symptoms more than 12 hours or high-risk non-STEMI ACS, an early invasive strategy is indicated for patients with any of the following:

  • Refractory ischemic chest discomfort

  • Recurrent or persistent ST deviation

  • Ventricular tachycardia

  • Hemodynamic instability

  • Signs of heart failure
